Cody Leeworthy, DPT
I received therapy for recovery from my left total shoulder replacement. I worked mostly with Cody Leeworthy DPT, Fred Wintermantel PTA, and their aides. Their approach was both professional and caring; and the entire department chipped in from time to time to keep the session moving. After three joint replacement surgeries and 40 years as an orthotist; I’ve observed many PT departments in various capacities. This experience was easily the best.
Thank you, Dr. Tom Toothaker, Josh, Bill and the entire team at OSC Physical Therapy. These folks really make excellence the standard! From the moment you walk in the door, there is no doubt that you are at the place you need to be to get your mobility back on track. Can’t say enough about this team. No doubt that I would visit them again.
I had the best experience possible when having shoulder surgery several months ago. Dr. Martin Coleman did an exceptional job diagnosing my shoulder issue, and laid out a plan to fix the torn rotator cuff, including removing bone spurs. I had limited pain and discomfort after surgery, and healed quickly. Dr. Coleman’s release to physical therapy decision early contributed to a quick recovery. Within four months after surgery, I’m able to return to most physical activities. I’m very thankful for the quality care I received from Dr. Coleman at OSC. Thank you, OSC and Dr. Coleman.
Over the years, Dr. Boyd Haynes has been our family’s orthopaedic surgeon. He has always provided truly remarkable skills and excellent care. His post-operative guidance in physical therapy also thoroughly supports the excellent outcomes that we have experienced. Our family is most appreciative.
Rehabilitation at the physical therapy arm of OSC was a very positive experience. So much so that I wish it could be extended into a continuing activity, with clients doing therapy alongside self-generated activities, much like local fitness and wellness facilities. Such a facility could be truly transformational. The business plan for such a facility would combine private and public insurance modalities and individual fees and deposits. Of course, the facility would need expansion and investment by OSC, but the payoff could be great for OSC and the local community.
My reasoning is based on the talent, organization and training evident in the staff behavior encountered during my therapy. First was the courtesy, competence, personal touches and thoughtfulness of all from entrance to exercise to departure, as well as attention to COVID requirements and cleanliness. Second, was the intangible quality of empathy so evident in the concern for client comfort. Encouragement and motivation were also demonstrated.
I could name many more, but Jim, Fred, Bill, Jonathan and Cody do deserve special mention, with regrets, I cannot add so many deserving others for lack of good memory and inattention. Suffice to say that I am thankful to all for this experience, which hopefully will bring my many years of enjoyable physical activity, similar to that depicted from 2011.
Cody Leeworthy, DPT
After seeing Dr. Jeffrey Carlson and getting an epidural shot to relive pain in my neck and shoulders, he set me up for 6 weeks of Physical Therapy with Cody Leeworthy, DPT and Josh Hancock, PTA along with some fantastic assistants. The more I did the exercises, the more relief I get. Everyone set me up for success and I want to thank all of you for the complete professionalism and courtesy I saw on each trip there. The care I have received, from seeing Dr. Carlson, to Dr. Raj Sureja, then over to PT, has been remarkable!!
Since my surgery November 10, 2020 with Dr. John Burrow, I have been going to Physical Therapy since Dec. 3, 2020. The Therapist was Dr. Cody Leeworthy and LPTAs Joshua Hancock and Jonathan Lorusso. I was seen mostly by Cody or Joshua. Each visit, I would discuss my areas of concern and they would listen and address to the best of their ability.
My exit patient survey was with Dr. Tom Toothacker and I informed him how all staff were professional and pleasant. He appreciated me speaking with him because as he stated “usually people only complain” and if need be, I should contact him if I need further assistance.
I have had pain in both knees for several years and had knee arthroscopies in the past. Even after I retired, I continued to work in my own lawn care business and the pain continued. I finally made an appointment with Dr. Boyd Haynes to see what could be done. He found bone-on-bone and told me that I would know when I needed to have knee replacement surgery. I was convinced and scheduled my first surgery for January of 2020. With Dr. Haynes’ expertise and my ,hard work with Physical Therapy (PT), I was back to normal walking, range of motion without pain.
This past January, I had my left knee replaced and again, with his skill and PT, I didn’t need a walker, just a cane at first, then he measured me and I had 0-degree extension and well over 100-degree flexion. He was pleased as was I, and am now working as before surgery, but without pain in my knees.
Even though I am 74 and a diabetic, I was able to maintain good Blood Sugar levels throughout.

Thank you, Dr. Haynes for your care and expertise in bringing me back to better mobility and pain free knees. My advice is to not wait too long and do what the doctor tells you, work hard and you, too, can, in return, have the knees of your youth!
Dr. Tom Toothaker is an excellent listener. He put a good plan in place for my treatment and placed me in the capable hands of Jonathan Lorusso, PTA. I believe the therapy pointed out exercises to help me at home to continue building the strength in my back.
I thank each of the assistants that helped me in my journey.
My name is Catherine Reid, I would like to thank Tom Toothaker, DPT. Joshua Hancock, LPTA and Fred Wintermantel PTA, for their care doing my rehabilitation at Orthopedic & Spine Center.
I was treated for pain in my left shoulder, which made it impossible to complete my normal physical duties and daily activities that I was accustomed to doing.
After eight weeks of rehabilitation, I have been able to resume all my my normal activities without pain. I was made comfortable at each session and was assisted with the best professional care and concern from each Therapist and their assistant.
I would recommend OSC to anyone needing Physical Therapy for any reason. I am truly thankful for the treatment, the healing and personal training that I received. Catherine Reid
