____ Medical Clearance Obtained (if needed)

____ Collar or Brace Fitted and Delivered

____ Collar or Brace Test-Driven

____ Pillows or Recliner for comfort

____ Comfort Items Purchased

____ Cough Drops, Chloraseptic Spray, Hard Sugar-Free Candy

____ Heating Pad, Tylenol, muscle rubs, ice packs

____ Personal Hygiene Items

____ Laxatives/Stool Softeners/Fiber Supplements

____ Dressing Changing Supplies – Gauze, tape, Band-aids, gloves

____ Comfortable Loungewear

____ Arrange Help with the Kids, Pets, Lawn, etc.

____ Arrange Transportation to and from the Hospital

____ Arrange Spouse/Family Member/Companion at Home for Early Recovery

____ Discontinue Prohibited Drugs 7 Days before Surgery

____ Pre-op Testing and Teaching Completed at Bon Secours Mary Immaculate Hospital

____ Contact Home Health care agency (if ordered)

____ Space made at home for home-assistive devices (bedside commode, walker, hospital bed, etc.)

____ Home-assistive devices delivered and set-up

____ Pack overnight bag for Hospital stay (if applicable):

• Pajamas or Nightgown, Bathrobe
